Volunteer Member Form Fields

In the hierarchy of how the form is built think of category as the umbrella. The next steps include adding the questions that are to be housed under the umbrella.  These items come together to make the enrollment form.


Category example - Basic Information

Sub items - 

  • Legal First Name
  • Legal Last Name
  • Email (Only if different than family email)
  • Gender


Using the instructions in the club member settings create the following categories, fields and answer options:


Category = Basic Information

Fields =

  • Legal First Name
  • Legal Last Name
  • Email (Only if different than family email)
  • Gender - Input type = Single Select with following options:
    • Male
    • Female
    • Prefer not to answer
  • Primary Phone
  • Are you a member of Collegiate 4-H (if your state tracks that). Input type = Single Select) with the following options
    • Yes
    • No
  • Are you an employee of X LGU (this is common if employees/staff/faculty serve as 4-H Volunteers). Input type = Single Select with following options:
    • Yes
    • No
  • Number of Years in 4-H as a volunteer
  • Did you participate in 4-H as a youth? Input type = Single Select with the following options:
    • Yes
    • No
  • If yes, number of years in 4-H as a youth
  • Mailing Address (if different than family mailing address)
    • Mailing Address Line 1
    • Mailing Address Line 2
    • Mailing City
    • Mailing State
    • Mailing Zip code
  • Emergency Contact Information
    • Emergency Contact Name
    • Emergency Contact Phone Number
    • Emergency Contact Relationship
  • Demographics (same as youth)
  • Military Service of Family (same as youth)
